What to bring to an appointment

  1. All W-2s, 1099s, and W-2Gs

  2. K-1 statements

  3. Year-end bank and mortgage statements

  4. Property tax statements for current and upcoming tax years

  5. 1095-A from the federal or state health care exchange

  6. 1095-B from your health insurance carrier or documentation
    of coverage for the year

  7. 1095-C from your employer-sponsored health insurance or documentation
    of coverage for the year

  8. All medical expenses

    • please categorize and calculate the amounts

  9. Health Saving Account (HSA) contribution and distribution amounts

  10. Brokerage statements original purchase information if not provided by your broker

    • (this includes the date of purchase, the cost, and expenses you incurred to hold
      the investments  including management or brokerage fees)

  11. Investment expenses for the current year

  12. 1098-T Higher education and scholarship/grant statements & billing transcript from
    school showing payments made during the tax year

  13. Student loan interest statement

  14. Home closing statement – if you sold a home bring the original home closing statement

  15. Income and expenses for any business

  16. Unreimbursed job expenses

  17. Dependent care provider name, address, ID, and amount you paid for each child

  18. Estimate tax payment records

  19. Bank account information for direct deposit (bank routing and account numbers

  20. Driver’s licenses or state ID for the taxpayer and spouse

New clients should also bring: 

  1. Copy of last year’s tax return
  2. Picture IDs for yourself, your spouse, and your dependents
  3. Social Security card or ITIN card for yourself, your spouse, and your dependents
